The main aesthetic problem after immediate implant placement (IIPP) is gingival recession and facial collapse on the buccal side of the implant. To avoid this problem, we propose grafting connective tissue without raising a flap, using a tunneling technique, in order to preserve the integrity of the hard and soft tissues. This technique has been used by the author for 17 years with good and very stable results. We show several cases and explain the reasoning of performing this approach.
